| Have an APPROVED VENDOR submit your acceptable performance evaluation (PE) samples results. The samples must be analyzed within the appropriate window*. |
| The following methods require acceptable PE results: |
- GRO by AK101 (water and/or soil)
- DRO by AK102 (water and/or soil)
- DRO by AK102-SV (water only)
- RRO by AK103 (soil only)
- BTEX by AK101/8021B/8260B (water and/or soil)

| Only analyze PE samples for the methods/matrices for which you are seeking approval and which require acceptable PE results. |
*Time windows
Initial Approval: PE Samples must be analyzed and results submitted not more than 90 days before and no later than 30 days after the initial application is submitted.
Renewal of Approval: PE Samples must be analyzed and results submitted between 90 to 30 days before expiration of approval to prevent a lapse of approval. |

Submit Electronic copies of:
1. Quality Assurance Plan containing the minimum elements listed in the USEPA Region 10
Guidance on Preparation of Laboratory Quality Assurance Plans.
2. Standard Operating Procedures (SOPs) for all methods for which you are seeking approval.
