Initial Fee for certification is $300. Recertification is $150. |
HOW TO APPLY FOR MICROBIOLOGY CERTIFICATION |
| Submit to the Certification Officer: |
- A completed application for the current fiscal year
- Laboratory Quality Assurance Plan
- Standard Operating Procedures of micro methods for which you are requesting approval
- Copy of Reporting Form to be used by the Laboratory
- A completed Personnel Survey form
- Performance Evaluation Data
- Applicable fees (money order or check) made out to the "State of Alaska".
